To Apply for Membership:
Applicants for Active or Affiliate Member Status
must enclose the following:

Payment of application fee and first year dues (see application
form). |
Applicants for Student Member Status must enclose
the following:

Completed application. |

Payment of first year dues (see application form). |
| • |
Student members pay no application fee. |
| • |
A letter from your program director on official letterhead
indicating date of expected graduation. |
Applications are considered by the Membership and Credentials Committee
throughout the year. Membership is contingent upon approval by the
